Please Note:

If you are using Internet Explorer, when you right click  your  mouse button (on the icon),  you may choose  "Open Link".    By choosing "Open Link" or "Open Link in New Window", the browser will automatically open "Word".    You can, of course, choose "Save Target As", in which case you are forced to save the file.   Whichever works best for you.      

If you are using Modzilla/Firefox browser, when you right click your mouse button (on the icon), you may choose "Open Link" in New Window.    The browser will automatically open "Word".   If you wish to save the form, choose "Save Link As" and you will be allowed to save your file in word format.

If you do not have "Word" on your pc, you will need to contact me  duganann@cox.net so that I can send you the file in .PDF format and you can use Adobe Acrobat Reader to display/print your document.   Adobe Acrobat Reader is a free utility which you can download onto your pc from the web.
